I hereby authorize Sevier County Utility District (SCUD) to initiate automatic debit entries from the financial instiution named below. Authorization is granted for payments of monthly utility bills with SCUD. Further, I agree not to hold SCUD responsible for any delay or lack of payment due to incorrect or incomplete information supplied by me or by my financial institution, or due to an error on the part of my financial institution.

This agreement remains in effect until SCUD receives written notice of cancellation from me or my financial institution or until I submit a new direct debit form to SCUD with 30 days notice. In addition, this agreement may be terminated by SCUD upon three consecutive returned draft notices.

Legal Information
The debit will occur monthly at the amount that appears on your bill. The first debit will begin on the due date of your first bill. Billing statements are mailed at least 21 days prior to debiting your account.

If the debit is recurring and the date of the debit falls on a non-banking day, the debit will hit your account on the next banking day and will not hit your account prior to the authorized date.
